29 CFR Part 1910.119(m) requires an investigation for "each incident which resulted in, or could reasonably have resulted in a catastrophic release of highly hazardous chemical in the workplace" It needs to be prompt, "not later than 48 hours following the incident." and the team needs to include "at least one person knowledgeable in the process involved."
